Do they really have to come out?
No, not as a matter of course. A wisdom tooth that is well positioned, fully through and cleanable has no reason to be removed. Taking out a healthy, functional tooth achieves nothing.
Removal is called for when the tooth is causing trouble, or is about to. The decision follows an examination and a radiograph — see our article: wisdom tooth removal, indications and recovery.
When removal is indicated
- Repeated pericoronitis — the gum partly covering the tooth becomes infected, swells and hurts. This is the most common reason.
- An impacted or partly impacted tooth that will never come through properly.
- Decay on the wisdom tooth or on the molar next to it, often out of the brush’s reach.
- Pushing at an angle against the neighbouring tooth, with a risk of resorption.
- A cyst or a lesion visible on the radiograph.
- Before orthodontic treatment, when there is not enough room.
The assessment before surgery
A panoramic radiograph shows the position of the tooth, how impacted it is and its relationship with the structures around it.
When the root appears close to the inferior alveolar nerve, 3D Cone Beam imaging is taken. It locates the nerve to the millimetre and lets the procedure be adapted. That is what separates a planned extraction from an improvised one.
How the removal is done
- Local anaesthetic. The procedure is painless. You stay awake; the area is numb.
- Access. For a tooth that is through, the removal is direct. For an impacted tooth, the gum is opened and a little bone released.
- Removal. An impacted tooth is often sectioned into two or three pieces: it can then come out through a smaller opening, so less bone is disturbed. That heals faster — it is not more serious.
- Sutures. A few stitches close the gum. They come out a week later, or dissolve on their own.

Recovery, honestly
The first 48 to 72 hours are the most noticeable: a swollen cheek, difficulty opening your mouth, sometimes a bruise. That is normal and it settles.
What genuinely helps:
- Cold on the cheek on day one, in 15-minute stretches.
- Painkillers as soon as the anaesthetic wears off, without waiting for the pain to set in.
- Lukewarm, soft food for the first few days. Nothing hot, no straws.
- No tobacco. It is the leading cause of dry socket — a painful inflammation of the socket that shows up around day three.
- Gentle brushing around the area, without avoiding it entirely.
Pain that increases instead of easing after day three is not normal: call the practice.
